Download Application Form 1003 and Associated Conformance Agreements

In order to expedite the loan approval process we have made available here a procedure to provide a copy of the loan application form and the required compliance forms.  Once these forms are received, we can usually respond with a loan approval within the same day.  NOTE: once the forms are completed and ready for delivery to us, please contact us for instructions concerning required supporting documentation (W-2, pay stubs, bank statement, etc.)

How do I download the forms with the Adobe© Acrobat Reader?
These forms are in a file format you can read and print using free Adobe Acrobat Reader software. Chances are you already have this software on your computer. If you don't have the Adobe Acrobat Reader, you can download and install the Adobe Acrobat Reader for free by clicking on this icon.   Get Adobe Acrobat Reader

Depending on the browser you are using, one of two things will happen when you click  the download selections below:

  • The requested forms will automatically open. If this happens, print the forms for your use.
  • or, you'll be asked if you want to Open or Save the file. Choose Open. When the file opens, print it for your use. (If the file does not open, save the file to your hard drive with the default name given. Then open and print it using your Adobe Acrobat Reader.)
     
The Loan Application Form can be downloaded by clicking here. This is a 3 page form on Legal size paper, so make sure you have 8 1/2 x 14 (legal size) paper in the printer when ready to print. (Note: if Legal Size paper is not available to you, it is acceptable for the form to be printed on 8.5 x 11 as a reasonable alternative.)

Once the application form has been downloaded and printed, it would be our pleasure to assist with it's completion.  Please call for assistance or to get instructions on what to do next. (NOTE: as an additional aid we have provided a sample application form on which we have highlighted the information needing your input.  You may download the sample form by clicking here, print it and use it for a guide.)

Before we can accept and act upon your application, we must have your signed and dated authorization on our form called Authorization to Release Information.  You may obtain that form by clicking here to download it, sign, date, and indicate your SSN on that form and return it along with the application form per the instructions below:

  • If equipped to do so, scan and electronically transmit the documents to us at mike@chasediversified.com

  • Return via US Mail or other courier of your choice using expedited delivery when time is of essence.

  • Fax the completed forms and supporting documentation to 954-428-3911 

  • Hand deliver to our office or arrange for us to meet you for personal delivery

Once you have completed, signed, and returned a copy of the Form 1003 and the Authorization to Release Information forms, it is necessary that we receive copies of certain support documentation, signed compliance forms, and originals of your application and Authorization.  The package of compliance forms is available by clicking here.  Please contact us for confirmation of required documentation and mailing instructions.
